A SMART Case: Stroke-like Migraine Attacks After Radiation Therapy

Stroke-like migraine attacks after radiation therapy is a rare syndrome with transient, reversible neurologic symptoms and imaging findings that often manifest with headaches and seizures, and occur years after radiation therapy for brain tumors. Although frequently mistaken on presentation for cerebral infarction, infection, or recurrent malignancy, radiologic features, including transient unilateral gyral enhancement and focal cortical edema, can indicate the diagnosis. Early consideration of this entity can prevent unnecessary treatment, including neurosurgical procedures, with potentially significant neurologic sequelae.
