Tacrolimus-Related Optic Neuropathy

Optic neuropathy has a broad differential diagnosis, including demyelinating disease, infection, and inflammatory etiologies such as sarcoidosis, radiation, and drug toxicity. Tacrolimus is a rare cause of optic neuropathy, but radiologists can benefit patients by suggesting the diagnosis. We report a case of tacrolimus optic neuropathy initially suggested on the basis of MR imaging findings. The patient improved clinically and had near resolution of MR imaging findings after treatment, which has not been previously reported.
