A case of a ruptured aneurysm at the internal carotid artery bifurcation, associated with an accessory middle cerebral artery will be presented along with a review of the literature describing variants of the middle cerebral artery. The variations in anatomy can influence the decision to proceed with endovascular or open surgical procedures.
